Definitely the coach--- ESPECIALLY since it seems that some type of game is lost in a similar situation to this on a yearly or every other year basis. I can think of at least 3 or 4 games whose vids made the internet rounds that were lost because the offense did not know you have to finish the last play of each quarter.

100% on the coaches. It wasn't one kid who stopped playing. All the kids stopped playing. Any time there's one guy not doing it right when everyone else is, it's probably on him. Any time everyone's not doing it right, it's on us.

Coaches Fault.
Good Idea of letting him run to the sideline, but in my eyes he should "scramble" to his team sideline, so the Coaches can tell him, when to step out of bounds!
