Does anyone have a sheet or form they use to hold exit interviews with players at the end of the year ?

What is the best part about football? What is the worst part of football? Anythings happen that you would struggle to explain to your parents? Like maple syrup shots? Are there any things you think the coaches could have done better or should change? What do you think we can do to get to the next level? What role do you expect to have next season? For seniors we talk about signing dates, scholarships, and otheer options. What is your winter activity? What is your plan for the spring? we don't have spring football Summer activities? We discuss recruiting and camps to attend for them and their aspirations. Are there any things that you think the coaches should be aware of or is there anything you would like to tell the coaches? Describe what a senior did to make your season better. Which underclassmen do you think possess the qualities to be a great leader?

I did them for the first time this year. Some kids had some really good insight, some kids it was a waste of time. You know the ones who have trouble looking you in the eye, communicating, and making complete sentences...
I generally followed this format Something positive about them and their performance (hard to do when you were 2-8) What do you think are your strengths? What are you good at? What do you think you need to work on most? What are your weaknesses? What do you enjoy about playing football? What do you not enjoy about playing football? Anything we do as a program that you just hate or dispise? What I think your strengths are What I think your weaknesses are and what you need to work on. If they were a graduating Senior I made sure to ask their plans for after graduation and thank them for the time they put into our program.

I have a Google Form I send my linebackers at the end of the season and then link it to a Sheet so I can have all the answers there. We then meet after a week and go over the answers. I then also use the same form for all my freshmen who return for their sophomore year and then go over their goals from their freshman year to see if they did what they set out to do. I'll copy the questions here:
Name (First Name, Last Initial) What were three things you felt you excelled at this season? What do you think is the biggest thing you have to work on, football-wise (i.e. Speed, Reads, Tackling, Weights, Studying Playbook, etc.)? What do you think is the biggest thing you have to work on, non-football wise (i.e. Competitiveness, Being a Leader, Being Unafraid to Speak Up, etc.)? Name some things our program does well, in regards to football. I'm talking about on the practice field or on gameday. Name some things, in regards to football, that our program could be better at. Name some things our program does well, in regards to preparation. This is for anything related to the weight room, team meetings, film study, etc. Not on the field, but impacts what happens on the field. Name some things, in regards to preparation, that our program could be better at. What do you promise to do this off-season to make us a better linebacker room, defensive unit, and team for next season? (List three things.)
